Job Summary
The Advanced Practice Nurse or Physician Assistant performs the role utilizing the patient-centered coordinated care model demonstrated through the following competencies: leadership, direct clinical practice, consultation/collaboration, coaching/guiding, research, and ethical decision making. This role is part of the stroke service at a comprehensive stroke center led by on sight vascular neurologists. We are seeking candidates for a night shift role.
Essential Job Functions:
1. Provides 24/7 patient care coverage for the stroke service. Performs E&M Inpatient consults and performs approved procedures under the supervision of attending of oversight.
2. Develops and implements appropriate plan of care.
3. facilitates patient teaching and support programs for patients.
4. Applies evidence and value-based practice protocols and standards of care that include interventions and treatments to achieve desired outcomes. Appropriately consults with attending of oversight for changes in plan of care.
5. Demonstrates effective communication and interdisciplinary skills.
6. Evaluates patient/family transition of care planning for resource utilization, cost,quality, and promotion of self-management for chronic health maintenance.
7. Documents effectively in patient charts through HMH EMR system.
8. Participates in continuing education and ensures compliance with CME requirements to maintain licensure.
9. Ensures compliance with stroke protocols and standards of care.
10. Other duties as assigned

Education, Knowledge, and Skills Required
1. Graduate of an accredited Physician assistant program or complete an NP-focused graduate master’s or doctoral nursing program.
2. Demonstrates effective communication skills
3. Strong computer skills and experience with EMR
License and Certifications Required:
1. Advanced Practice Nurse or Physician Assistant License
2. AHA Basic Health Care Life Support Certification (BLS); ACLS preferred in addition to BLS.
Job Type: Full-time
Pay: $120,000.00 – $160,000.00 per year
